Background Autism in adulthood is characterized by marked heterogeneity in cognitive, socio-emotional, and affective functioning. Attention-deficit/hyperactivity disorder (ADHD) frequently co-occurs with autism, contributing to diagnostic complexity and variability in clinical presentation. Methods Trait-based dimensions derived from self-report screening instruments (AQ, EQ, ALS-18, BAARS-IV) and WAIS-IV cognitive indices were examined in 164 adults diagnosed with autism, ADHD, or co-occurring autism + ADHD. Stepwise discriminant function analysis was used to identify multivariate profiles differentiating these clinical presentations. Results Two significant discriminant functions emerged. The first (87.6% variance) was primarily defined by self-reported empathy (EQ) and autistic trait severity (AQ),distinguishing autistic from ADHD presentations. The second (12.4% of variance) was characterized mainly by working-memory performance (WMI), current ADHD symptoms (BAARS-6), and affective lability (ALS-18), contributing to the differentiation of adults with co-occurring autism + ADHD. The discriminant model correctly classified 65.2% of the original cases and 61.6% of cases following leave-one-out cross-validation. Conclusions The findings support the value of combining cognitive indices with dimensional trait measures to characterize heterogeneity across adults with ADHD, autism, and co-occurring autism + ADHD. Although the discriminant model identified clinically meaningful multivariate profiles, its moderate cross-validated classification accuracy indicates that it should be regarded as exploratory and requires replication and external validation before clinical application. |
