This chapter provides a history of ASD and Asperger’s Syndrome, and discusses the evolving meanings of the autism diagnosis over the decades. It reviews the sometimes controversial roles of those, including Hans Asperger and Leo Kanner, who raised awareness of the condition. The authors discuss the diagnostic changes in each edition of the DSM and how the ASD diagnosis has taken a journey from a loosely identified set of conditions, to a severe syndrome caused by parents, to a biological illness needing a cure, to an identifiable spectrum of characteristics.

Distinguishing between symptoms of ASD and symptoms of other disorders can be a challenge. Chapter 3 provides a brief overview of diagnoses that may present as ASD but are not, such as attention deficit/hyperactivity disorder, intellectual disability, and oppositional defiant disorder. Clinicians should keep in mind that the disorders discussed in this chapter are only a few of the psychiatric disorders that have symptoms that can overlap with symptoms of ASD. This chapter will focus on symptoms that seem similar across disorders as well as ways in which clinicians may distinguish between disorders. It is important to consult the DSM for other differential diagnoses. The chapter discusses how the diagnoses are similar and different to ASD and provides vignettes as examples.

It is critical that clinicians are informed about and trained in proven, research-driven, family-based treatments to ensure children and their families are receiving optimal care. As ASD impacts both the child and his/her family, providing parents with specific techniques to manage behavior across settings is critical in the treatment approach. Parent-child interaction therapy is a family-based model originally developed to reduce challenging behaviors while improving caregiver-child relationships for children without ASD. However, accumulating research has shown parent-child interaction therapy, without major adaptation and while maintaining its core components, produces comparable outcomes with children on the autism spectrum by ameliorating behavioral challenges and reducing caregiver stress. This chapter provides a clinical and research overview of parent-child interaction therapy, a theoretical rationale outlining the utility of the treatment with the ASD population, and examples of clinical modifications of the model tailored for the ASD population and their families.

Using information gained from a focus group study, which investigated the financial planning needs of special needs parents and caregivers, the authors of this chapter discuss barriers and challenges of financial planning for loved ones as reported by study participants. Résumé :

This chapter will focus on an introduction to augmentative and alternative communication for children with ASD. Augmentative and alternative communication provides additional modes or methods for children to communicate in addition to or sometimes in place of spoken words. The authors of Chapter 26 will review various methods that help individuals with ASD communicate such as low-tech systems (e.g., manual sign and the picture exchange communication system) and high-tech devices (e.g., speech generating devices and smart devices). The chapter also reviews key considerations for selecting the communication type such as the type of language that the child already uses and the complexity of the child’s communication.

Parents of children with ASD are at increased risk of poor psychological well-being because of the unique parenting challenges and cultural stigma they are often faced with. This chapter reviews scholarship from the authors’ own research group, as well as that of others, on four aspects of self-care – sleep, social support, partner relationship quality, and positive emotion and self-kindness – found to be related to positive psychological well-being in parents of children with ASD. Each section ends with recommendations for clinicians. Self-care strategies that may be of benefit to parents of children with ASD include focusing on good sleep quality, cultivating effective social support, attending to the partner relationship quality, and fostering positive emotion and self-kindness. Each of these self-care strategies has been linked to better psychological well-being in parents of children with ASD.
