Males, particularly under 12 months of age, were especially vulnerable. Infections such as meningitis and encephalitis, particularly among aboriginal children, were responsible for over half the cases and accidents were responsible for one-quarter. Other causes included epileptic fits and cerebrovascular accidents. There was a positive association with parity and neonatal problems and a negative one with birthweight, suggesting that children 'at risk' neonatally are more susceptible to a postnatal brain-damaging event. Preventive programmes should include improvement in aboriginal living conditions; legislation for car restraints for infants; and health education for parents on the vulnerability of infants, the importance of immunisation, and how to recognize early the signs of potentially serious infections. This information may be useful in estimating the crown-rump length, and hence the gestational age, of advanced human embryos and early fetuses when direct measurement is impossible. Five widely-spaced levels along the spinal cord were studied, but only those data relevant to the levels of the first thoracic (T1) and twelfth thoracic (T12) vertebral bodies are described in detail. Similarly, although eight distinct spinal-cord dimensions were measured at each level, only those measurements that bore a high enough correlation with crown-rump length to be of any predictive value are presented here, the most useful being the maximum spinal-cord width and the maximum grey-matter width, both of which increased with gestational age in a highly predictable way. The descriptive account of the morphological appearance of the cord at these levels also enables, though less accurately, an estimate of the crown-rump length of a conceptus to be made by direct inspection of a single transverse section through the thoracic region of the cord. These results were further substantiated by testing the sucking rate on an empty and full breast, and the sucking pattern of an infant with a non-lactating mother. The study shows that there is no dichotomy between two sucking rates, but rather a continuous variation in response to milk-flow rate. Although the hair-shaft abnormalities may be seen with light microscopy (LM), they are best visualised with scanning electron-microscopy (SEM). Pili torti may be mistaken for monilethrix by LM, but SEM shows the true defect. A review of the literature shows that these hair-shaft abnormalities (trichorrhexis nodosa, monilethrix and pili torti) are not specific or pathognomonic, but do indicate a group of neurological disorders, including potentially treatable inborn errors of metabolism. The asymmetrical tonic neck reflex, tonic labyrinthine reflex-supine, and Moro reflexes were assessed for each child at birth and at intervals up to 12 months. Ages of rolling prone to supine, rolling supine to prone, and sitting alone were elicited from parents. The effects of the primitive reflexes on early motor activity were assessed, and statistically significant correlations were demonstrated between decreased reflex activity and the emergence of motor milestones. The distinctive association of reflex activity with motor function suggests the interaction of several reflexes (a primitive reflex profile) rather than the influence of isolated reflex activity. Such patterns support the hypothesis that decreasing primitive reflex activity is associated with the onset of volitional motor activity in normal infants. The asymmetrical tonic neck reflex, tonic labyrinthine reflex-supine, and Moro reflexes were assessed for each child at birth and at intervals up to 12 months. Ages of rolling prone to supine, rolling supine to prone, and sitting alone were elicited from parents. The effects of the primitive reflexes on early motor activity were assessed, and statistically significant correlations were demonstrated between decreased reflex activity and the emergence of motor milestones. The distinctive association of reflex activity with motor function suggests the interaction of several reflexes (a primitive reflex profile) rather than the influence of isolated reflex activity. Such patterns support the hypothesis that decreasing primitive reflex activity is associated with the onset of volitional motor activity in normal infants. The specific historical factors and behavioural profiles found to occur with high frequency among the malnourished children are discussed. They suggest a common etiology with other disorders of attachment--child abuse, nonorganic failure to thrive, and the 'vulnerable child' syndrome. These commonalities are explored, and possible mechanisms in the emergence of bonding failure are considered.
