THOMAN, Auteur Année de publication : 1983 Article en page(s) : p.283-296 Langues : Anglais (eng) Index. décimale : PER Périodiques Résumé : The sleep states of nine premature infants were observed and concomitant respiration recordings were obtained at 37 and 41 weeks conceptional age. 19 fullterm infants were also studied at 41 weeks conceptional age. Between 37 and 41 weeks significant decreases occurred in the premature infants' percentage of periodic apnea, rate of apnea in active sleep-REM, and length of the longest apneic episode. The premature and fullterm infants at 41 weeks did not differ in the frequency of apneic episodes, but the premature group had shorter episodes. These results provide additional evidence for the difference between premature and fullterm infants in central nervous system organization. Titre : Sleep behavior of preterm children at three years of age Type de document : Texte imprimé et/ou numérique Auteurs : Judy A. UNGERER, Auteur ; Marian SIGMAN, Auteur ; Leila BECKWITH, Auteur ; Sarale E. COHEN, Auteur ; Arthur H. PARMELEE, Auteur Année de publication : 1983 Article en page(s) : p.297-304 Langues : Anglais (eng) Index. décimale : PER Périodiques Résumé : Caregivers of 50 preterm children completed a questionnaire relating to their child's sleep behavior at three years of age. Assessments of the children's EEGs at term and three months, medical status at birth and at four, nine and 24 months, caregiver-child interaction at one, eight and 24 months, the Bayley Scales at 25 months and the Stanford-Binet at five years of age were also obtained. Sleep disturbances were frequently noted at three years of age. However, by five years of age, the number of reported sleep-problem children was greatly reduced, and they were not the same children with sleep problems at three years. Surprisingly, children with the greatest difficulty falling asleep at night at three years were likely to have had fewer medical problems, higher scores on the Bayley scales, and more positive social interactions with their caregivers in the first two years. The relationships among early physical, psychological and social development and sleep behavior are discussed. Titre : Outcome at school-age after neonatal mechanical ventilation Type de document : Texte imprimé et/ou numérique Auteurs : Tania R. GUNN, Auteur ; Elizabeth LEPORE, Auteur ; Eugene W. OUTERBRIDGE, Auteur Année de publication : 1983 Article en page(s) : p.305-314 Langues : Anglais (eng) Index. décimale : PER Périodiques Résumé : 103 school-age children (5 to 12 years) who survived mechanical ventilation for neonatal respiratory failure were evaluated for growth, neurological, intellectual, psychological and school function in order to determine those children most at risk for handicap. A major handicap occurred in seven children, preventing attendance at normal school or normal classes. Neurological sequelae were significantly associated with perinatal asphyxia and with birthweights of 1500g or less, and neurological sequelae and socio-economic factors were the major determinants of ability. The effects of the Neonatal Intensive Care Unit (NICU) experience on parents and subsequent parent-child relationships were also investigated: 67 per cent of the mothers were very upset by the experience and many continue to worry excessively about the health of their child. Parents who visited their child in the NICU frequently were significantly more anxious and overprotective, restricting many activities even when the child was of school age. Titre : Perceptual, motor and attentional deficits in seven-year-old children: neurological and neurodevelopmental aspects Type de document : Texte imprimé et/ou numérique Auteurs : Peder RASMUSSEN, Auteur ; Christopher GILLBERG, Auteur ; Eva WALDENSTROM, Auteur ; Bertil SVENSON, Auteur Année de publication : 1983 Article en page(s) : p.315-333 Langues : Anglais (eng) Index. décimale : PER Périodiques Résumé : This study presents the results of neurological assessments of a representative sample of seven-year-old Swedish children with perceptual, motor and attentional deficits and their controls (N = 141). Among those children diagnosed as suffering from minimal brain dysfunction (MBD), the majority showed neurodevelopmental deviations indicative of the "clumsy child syndrome'. However, 20 per cent had slight signs of choreoathetosis, diparesis, hemiparesis or ataxia. Other associated neurodevelopmental deviations are considered. The correlation between neurological findings and background factors is discussed: the MBD group had higher scores for various "organic' background factors than the comparison children, and the children with neurological syndromes had somewhat higher scores than the remaining children with MBD. The difficulties in distinguishing MBD from mental retardation, cerebral palsy and childhood psychoses is clearly illustrated. Long-term follow-up will indicate the prognostic significance of these findings. Permalink : https://www.cra-rhone-alpes.org/cid/opac_css/index.php?lvl=notice_display&id=564

In 307 studies in which the shunt was identified as nonfunctional, 208 patients were symptomatic for increased intracranial pressure and had the shunt revised. Of the other 99 patients, 71 have remained asymptomatic for three to 60 months (mean 27 months) and 28 subsequently required shunt revision after one to 67 months (mean 24 months). Five other clinical patterns are described. Various methods of monitoring shunt function are recommended. Permalink : https://www.cra-rhone-alpes.org/cid/opac_css/index.php?lvl=notice_display&id=564

ROSENBLUM, Auteur ; David N. FINEGOLD, Auteur ; Edward B. CHARNEY, Auteur Année de publication : 1983 Article en page(s) : p.338-342 Langues : Anglais (eng) Index. décimale : PER Périodiques Résumé : The lengths and arm spans of 99 children with myelomeningocele were measured. 52 of the children had short stature (less than the 3rd percentile): this occurred in 80 per cent of those with lesions at L3 or higher and in 43 per cent of those with L4-L5 lesions. Stature was unaffected among children with S1 or lower lesions. 54 per cent of all the children had an arm span wider than the anticipated range for length; of these, 69 per cent also had short stature. It is concluded that short stature is common among children with myelomeningocele, particularly those with the highest spinal lesions, and that arm span may be a more useful measure than length in evaluating growth velocity of these children. Titre : Thin ribs on chest X-ray: a useful sign in the differential diagnosis of the floppy newborn Type de document : Texte imprimé et/ou numérique Auteurs : John P. OSBORNE, Auteur ; E. Gordon MURPHY, Auteur ; Alan HILL, Auteur Année de publication : 1983 Article en page(s) : p.343-345 Langues : Anglais (eng) Index. décimale : PER Périodiques Résumé : The significance of thin ribs among 26 newborns diagnosed as having neuromuscular disease is discussed. 19 had a chest X-ray. Thin ribs were observed in three of seven with congenital myotonic dystrophy, in two of four with myotubular myopathy and in one of five with spinal muscular atrophy. The ribs were normal in one case of nemaline myopathy and two cases of non-specific myopathy. Polyhydramnios was recorded in four of the six cases with thin ribs. Ventilatory support frequently was required for these infants. The observation of thin ribs on X-ray is highly suggestive of neonatal neuromuscular disease, and is neither specific for nor invariable in congenital myotonic dystrophy. While the red facial rash (adenoma sebaceum) is taken to be one of the classic symptoms, depigmented skin macules were more common. Analysis of effect-size information derived from the studies revealed that the average child receiving drug treatment was less hyperactive than 88.5 per cent of the control children. 30 per cent of this effect could be attributed to placebo phenomenon. The analysis also indicated that drug therapy had the most pronounced effect on dependent variables consisting of standardized or informal measures of behaviour and the least effect on measures of IQ and academic achievement. The advantages of quantitative reviewing methodology for adding clarity and consensus to complex bodies of literature are demonstrated and discussed. This identical presentation in twins supports the view that the site of tumour formation is determined during intra-uterine development.
