The median age at operation was 14 years. 29 patients were found to have mesial temporal sclerosis or sclerotic hemispheres and 17 had alien tissue and one sclerosis with heterotopia. No lesion was found in three cases. The nature of the first seizure was found to relate significantly to pathology. 30 patients had suffered an early convulsion. Between the convulsive insult and the onset of focal epilepsy there was a silent interval, the duration of which was biased by sex and side of lesion. The remaining 20 patients, of whom a highly significant number had alien tissue, had not had an early convulsion before the onset of focal epilepsy. The importance of accurate clinical histories is stressed. Clinical considerations alone predicted the pathology and laterality of the lesion in more than half the series. Regular EEG recordings and repeat scanning were found to be important investigations. Major disorders of behaviour had occurred in 44 patients. As adults, the series were free of severe psychiatric symptoms, but many required years of careful rehabilitation. 31 children had been excluded from normal schools before operation. Postoperatively, many patients continued their education in normal schools. Some required basic remedial teaching, others successfully undertook higher education and training. 49 patients benefited over-all from neurosurgical treatment. 34 had no more habitual epilepsy at all. Speed of walking and heart rate were monitored continuously, both barefoot and in shoes, over eight consecutive lengths of a 25m walkway. Since heart rate is affected by speed, the physiological cost index (PCI)--which combines the two parameters--was calculated for each child, both barefoot and in shoes. Speed tends to increase and walking heart rate to decrease with age. However, resting heart rate also decreases with age, so the PCI has the same range for all ages. The mean PCI in shoes was 0.4 and barefoot was 0.38. The PCI for children with pathological gait is appreciably greater than that for normal children, so the PCI is a valuable quantitative indicator of the level of handicap. Electron microscopic analyses showed that the resulting CNS distortions invariably included paired spinal-cord tubes that developed ciliated central canals surrounded by cellular layers and rimmed laterally by axon-filled marginal zones. Specific axon tracts grew in appropriate locations through the regions of simple diastematomyelia. Even in the most histologically distorted regions, ventral roots developed, axons were myelinated, and neuromuscular synapses formed in adjacent muscles. On the other hand, an excess of cells populated the defects and in histologically distorted regions the axon tracts were clearly less well organized than in normal animals. Neural-crest cells and the notochord did not appear to be affected. In addition, distant neurons and axon tracts appeared normal. These mechanical lesions could produce spina bifida without hydrocephalus. It is concluded that mechanical lesions to the forming neural tube will produce mainly local effects with a strong tendency toward diastematomyelia.
