The prevalence of cerebral palsy (CP) diagnosed by age six years was compared for groups who had various antenatal and perinatal attributes. For children who had weighed less than 2000g the CP prevalence was 3.6 per cent for those less than 1500g and 4.3 per cent for those between 1500g and 1999g. The main factors predicting CP in the low-birthweight infants were measures of neonatal asphyxia, but the majority of children with CP had no extra adverse perinatal risk-factors compared to non-CP survivors, were not asphyxiated and had not received high-level neonatal intensive care. The CP rates were not significantly different between any other available antenatal, perinatal or sociodemographic risk-factor groups. It is suggested, therefore, that CP is not a good index of the impact of neonatal intensive care, and that research should now concentrate on elucidating the genetic, developmental and antenatal risk-factors for the various CP syndromes. No improvement of MDI scores of VLBW survivors in the 1980-82 cohort could be attributed solely to perinatal care. The two-year-old VLBW children in the 1980-82 cohort had similar rates of sensorineural impairments, disabilities and mean MDI to those who would have survived with the care available in 1966-70. It is concluded that survival of VLBW infants has improved in recent times but that neurodevelopmental outcome still lags behind that of normal-birthweight peers. Of the 120 infants, 47 presented with only minor perinatal risks: these had normal neurological and developmental findings in infancy and at follow-up and they formed the control group. 73 infants initially had developmental delay or abnormal neurological findings: at follow-up 35 were normal and 38 had pathological conditions (16 general psychomotor retardation, 15 cerebral palsy, four degenerative and three neurometabolic diseases of the CNS). Marked prolongation of N1 peak latencies (greater than 2ms) above the two-sigma border, or absence of potentials on one or both sides, indicated handicap after their first birthday in all 19 infants showing such alterations.
