The probabilities for neurodevelopmental disorders were calculated according to the ultrasound findings. The results showed that the probability of a major or minor disorder was low for infants whose scans did not show periventricular haemorrhage or markedly increased parenchymal echodensities in the first week of life, and for those whose scans at discharge gave no evidence of ventricular dilatation, hydrocephalus or cerebral atrophy. By contrast, the probability of a disorder was very high for infants with markedly increased parenchymal echodensities in the first week, and for infants with evidence of cerebral atrophy at discharge. The majority of the infants could be assigned, on the basis of the ultrasound scan at discharge, either to a large group who were at low risk of neurodevelopmental disorders or to a small group who were at high risk; the remainder were at intermediate risk. These findings may be used as a guide to the prognosis for other infants whose ultrasound scans show similar appearances. The children were given a comprehensive examination on entry to school, the findings from which are briefly described. Here the results of the neurodevelopmental assessment are reviewed. Children with a high (abnormal) neurodevelopmental score (NDS) are shown to be a very vulnerable group with significantly higher rates of learning difficulty by the age of 10. The NDS alone does not give as good a prediction of such difficulty as the school doctor's clinical judgment. It is emphasised that prediction of difficulties is not an objective of the school entrant examination but clearly the clinical tests from which a NDS can be calculated provide a sound basis for clinical practice. Attention is drawn to the need for the school doctor with discretion to pass on to teachers the results of their examination that may increase the teachers' insight into their pupils and help them in their methods of teaching and care. There were eight boys and two girls, ranging in age from eight months to 2 1/2 years at diagnosis. Eight of the 10 had evidence of oral motor planning problems which significantly affected their speech output. Five had evidence of awkwardness or clumsiness, suggesting difficulties with gross motor organization as well. This concurrence of motor planning problems in at least two spheres, ocular motor and oral motor, suggests more general difficulties with motor organization in this syndrome. In all cases for whom data were available the ocular motor problems resolved but oral motor problems persisted.
