Son diagnostic est particulièrement difficile. Le moment diagnostique, qui représente la genèse de la maladie pour le patient se situe au croisement d’une multitude d’éléments. Il est empreint des classifications des maladies, en l’occurrence du DSM puisque nous nous plaçons dans le champ des maladies psychiatriques mais il est également influencé par l’expérience, le regard du médecin face au discours et aux signes de son patient. De plus, le diagnostic d’un trouble comme l’autisme est en proie aux évolutions sociales et culturelles qui viennent sans cesse modifier le curseur entre le normal et le pathologique notamment dans le cadre des maladies mentales qui se veulent particulièrement sensibles aux mouvements de la société. L’autisme infantile est un trouble singulier qui va permettre de faire ressortir toute l’ampleur de ces problématiques épistémologiques. Trouble récent, qui a connu de nombreuses modifications au sein même des nosologies, l’autisme apparaît encore aujourd’hui comme difficile à cerner en raison, notamment, du large spectre de symptômes possibles qu’il recouvre. Cet article propose donc une analyse philosophique du phénomène et souligne la nécessité d’une réflexion philosophique autour de la question diagnostique.
