Results: The results showed that through the Autism Diagnostic Observation Schedule it is possible to assess the socio-relational key elements on which the therapy is based. There was evidence, in fact, of significant improvements after two and four years of therapy, both for children with severe autistic symptoms and for those in autistic spectrum. Conclusions: Socio-relational aspects represent the primary element on which work in therapy with autistic children and can be considered as indicators of a positive evolution and prognosis that will produce improvements even in the cognitive area. Moreover, greater initial social impairments led to relatively poorer outcomes, potentially suggesting that children with greater social affect difficulties may require a higher dose, or modified intervention, to that used in this study. Besides, it addresses the emerging pathogenic, etiologic, and diagnostic clues of diverse origins, encompassing genetic-, neurotransmitter-, androgenic- and immune inflammatory-anomalies in ASD patients, as availed by advances in Omics technologies (like genomics and proteomics). Moreover, as unravelled by metabolomics studies, metabolic flaws that pertain to amino acids, fatty acids, mitochondrial anomalies, and oxidative-stress are highlighted and further correlated with the extent of clinical picture and mal behaviors coherent with ASD patients. There are no treatments that have demonstrated evidence of being effective at improving social skills and promoting independence. The transition to adulthood, which often involves loss of school support and child and adolescent mental health services, is a challenge in the rehabilitation field. Our pilot study is the first that describes a novel approach to involving young adults with ASD in metacognitive exercise focused on social cognition. In treatment proposed, participants with ASD help each other to create software aimed at improving children with ASD’s understanding of emotions. Our results showed an qualitative and quantitative improvement of their cognitive empathic abilities, as witnessed by parents and of the post- treatment assessment. This pilot study highlights the importance to help individuals with ASD to fulfil their potential in areas of strength. With highly specialized imaging technology revealing functional and structural abnormalities in the brains of patients with ASD, the role of routine structural conventional Magnetic Resonance Imaging (MRI) is controversial and may not be a useful tool for the purpose of diagnosis and treatment. This study evaluated the utility of conventional routine MRI in ASD with and without co-occurring epilepsy. We reviewed the records of children with ASD ages 8-17 years and performed statistical analysis comparing MRI brain abnormalities and epilepsy. Seventy-seven of 253 subjects underwent brain imaging, 55 had MRI results available and, of these, 8had brain parenchymal abnormalities. These abnormalities included two subjects with Chiari I malformation, one each with hamartoma, enlarged vascular space, venous angioma, and three with abnormal white matter signals. Brain MRI parenchymal abnormalities were not associated with the presence of epilepsy in children with ASD. In our cohort, conventional routine MRI was not useful as a tool that could lead to immediate diagnosis or treatment. The benefit and risks of conventional brain MRI for children with ASD should be carefully weighed. If an imaging study is highly suggested, some of the clinically available advanced imaging technologies should be considered in place of conventional MRI. With highly specialized imaging technology revealing functional and structural abnormalities in the brains of patients with ASD, the role of routine structural conventional Magnetic Resonance Imaging (MRI) is controversial and may not be a useful tool for the purpose of diagnosis and treatment. This study evaluated the utility of conventional routine MRI in ASD with and without co-occurring epilepsy. We reviewed the records of children with ASD ages 8-17 years and performed statistical analysis comparing MRI brain abnormalities and epilepsy. Seventy-seven of 253 subjects underwent brain imaging, 55 had MRI results available and, of these, 8had brain parenchymal abnormalities. These abnormalities included two subjects with Chiari I malformation, one each with hamartoma, enlarged vascular space, venous angioma, and three with abnormal white matter signals. Brain MRI parenchymal abnormalities were not associated with the presence of epilepsy in children with ASD. In our cohort, conventional routine MRI was not useful as a tool that could lead to immediate diagnosis or treatment. The benefit and risks of conventional brain MRI for children with ASD should be carefully weighed. If an imaging study is highly suggested, some of the clinically available advanced imaging technologies should be considered in place of conventional MRI. These children are parent dependent and presence of parent during dental treatment is mandatory. Care must be taken of any casual remarks to the parent or assistant in front of child, the waiting room should have play objects, after one or two social visits treatment procedures can be introduced. They must be simple at first and are usually confined to the prophylactic polishing. The traditional method is to ask the patient to raise his hand if he wishes the dentist to pause. Toothpaste is produced and discussed as a familiar topic, dentist’s finger nail are then polished first, the parent’s and then finally child’s. From here it may be possible to polish the anterior teeth then followed by bur by introducing special small brush for posterior teeth, care should be taken to make no unexpected movements. Initially, simple restorations should be done followed by multiple restorations. If the child is not cooperative for the treatment, it has to be done under sedation.
