Based on results of the functional assessments it was hypothesized that appropriate and disruptive mealtime behaviour was maintained by different contingencies. A non-concurrent multiple baseline design across participants was utilized to validate the effectiveness of the intervention. Intervention for all participants included presentation of food on a spoon for 30 s unless acceptance occurred. Acceptance resulted in putative reinforcement. The meal ended after 20 presentations. For all participants, acceptance and mouth cleans increased while disruptive behaviour decreased, and effects were maintained at follow-up. An assessment was implemented with eleven individuals to identify the specific instructional variables influencing the individual's responding. A prescribed academic intervention was selected for participants based on the results of the functional assessment. The prescribed intervention was compared to an alternative treatment and control condition. The functional assessment identified several different patterns of responding to instructional variables across participants. The treatment evaluation demonstrated that the prescribed academic intervention was effective in teaching conditional discriminations. It is common for clinicians to assume that repetitive behaviors are maintained by automatic reinforcement but, as with any challenging behavior, the function of stereotypy should not be assumed based on its topography. Previous research on the functional assessment of stereotypy has produced mixed results, and most previous studies on the functional assessment of stereotypy in children with ASD have included small sample sizes, so more research in this area is warranted. In the current study, indirect functional assessments consisting of the questions about behavioral function (QABF) were conducted across 53 children and adolescents with ASD. The QABF produced interpretable results in a high percentage of cases and 90% of the stereotypic behavior appeared to be maintained by automatic reinforcement. STARK, Auteur ; Julia C. BARNES, Auteur ; Nicholas D. YOUNG, Auteur ; Robin L. GABRIELS, Auteur Article en page(s) : p.3468-3474 Langues : Anglais (eng) Mots-clés : Autism spectrum disorder Children Adolescents Crisis behaviors Functional assessment Psychiatric hospital care Index. décimale : PER Périodiques Résumé : Children and adolescents with autism spectrum disorder (ASD) are at risk for emotional dysregulation and behavior problems that can escalate to levels requiring psychiatric hospitalization. Evaluating the etiology of such behaviors can be challenging for health care providers, as individuals with ASD can have difficulty self-reporting concerns. This brief report introduces the Iceberg Assessment Interview (IAI), a tool to organize and elucidate the assessment of issues potentially underlying problem behaviors. A summary of IAIs from a chart review of patients ages 5–18 with ASD (n = 23) admitted to a specialized psychiatric hospital unit illustrates the clinical utility of this tool. Summarized IAI data includes presenting crisis behaviors, caregiver-perceived environmental conditions, and underlying psychosocial and medical problems.
