We investigated the experiences of children with ASD as victims and/or perpetrators of bullying. Children with ASD epidemiologically ascertained participated in a cross-sectional study. Although children with ASD showed significantly increased risk for bullying involvement compared to community children, after controlling for comorbid psychopathology and other demographic factors, increased risks for being perpetrators or victim-perpetrators disappeared while risk for being bullied/teased continued to be significantly elevated. This finding will help guide medical, educational and community personnel to effectively identify children with ASD at risk for school bullying and develop interventions. Use of automated facial expression detection technology enables accurate and efficient tracking of facial expressions that has potential to identify individual response differences. METHODS: Children and adults with ASD (N = 124) and typically developing (TD, N = 41) were shown short clips of "funny videos." Using automated facial analysis software, we investigated differences between ASD and TD groups and within the ASD group in evidence of facial action unit (AU) activation related to the expression of positive facial expression, in particular, a smile. RESULTS: Individuals with ASD on average showed less evidence of facial AUs (AU12, AU6) relating to positive facial expression, compared to the TD group (p < .05, r = -?0.17). Using Gaussian mixture model for clustering, we identified two distinct distributions within the ASD group, which were then compared to the TD group. One subgroup (n = 35), termed "over-responsive," expressed more intense positive facial expressions in response to the videos than the TD group (p < .001, r = 0.31). The second subgroup (n = 89), ("under-responsive"), displayed fewer, less intense positive facial expressions in response to videos than the TD group (p < .001; r = -?0.36). The over-responsive subgroup differed from the under-responsive subgroup in age and caregiver-reported impulsivity (p < .05, r = 0.21). Reduced expression in the under-responsive, but not the over-responsive group, was related to caregiver-reported social withdrawal (p < .01, r = -?0.3). LIMITATIONS: This exploratory study does not account for multiple comparisons, and future work will have to ascertain the strength and reproducibility of all results. Reduced displays of positive facial expressions do not mean individuals with ASD do not experience positive emotions. CONCLUSIONS: Individuals with ASD differed from the TD group in their facial expressions of positive emotion in response to "funny videos." It has shown promise as a screening measure for autism spectrum disorders (ASDs) in a research-referred older sample, though recent studies with younger children reported lower sensitivities when using the suggested cutoff of ≥15 to differentiate ASDs from children with nonspectrum disorders (NS).
Methods: Diagnostic discrimination of the SCQ was evaluated alone and in combination with the ADOS (Autism Diagnostic Observation Schedule) in a clinical and research-referred sample of 590 children and adolescents (2 to 16 years), with best estimate consensus diagnoses of autism, pervasive developmental disorder, not otherwise specified (PDD-NOS) and non-ASD disorders. The SCQ was completed before the evaluation in most cases. Performance of the SCQ was also compared with the Autism Diagnostic Interview – Revised (ADI-R).
Results: Absolute scores and sensitivity in the younger children and specificity for all groups were lower than reported in the original study. Using receiver operating curves (ROC) to examine the area under the curve (AUC), the SCQ was more similar to the ADI-R total score in differentiating ASD from NS disorders in the older (8–10, >11) than younger age groups (<5, 5–7). Lowering the cutoff score in the 2 younger groups improved sensitivity, with specificity remaining relatively low in all groups. Using the SCQ in combination with the ADOS resulted in improved specificity. Diagnostic discrimination was best using the ADI-R and ADOS in combination.
Conclusions: Those interested in using the SCQ should consider adjusting cutoff scores according to age and purpose, and using it in combination with another measure. Methods Two large-scale South Korean epidemiologic samples were used to examine whether completion of childhood vaccinations decrease likelihood of having ASD as assessed by Autism Spectrum Screening Questionnaire (ASSQ) and behavioral problems scores. Parental reports on vaccination completion were categorized in groups: <3, 4?5, & 6. The primary outcome is the likelihood of having ASD and/or, internalizing, and externalizing behavioral symptoms. Likelihood of having ASD was categorized as: low (ASSQ < 10), intermediate (ASSQ = 10–14), and high (ASSQ ? 15). The risk for externalizing/internalizing symptoms was assessed with the Behavior Assessment System for Children-Parent Rating Scale. We examined the hypothesis in a Discovery Sample (DS) (N = 10,006) and verified findings in a Replication Sample (RS) (N = 29,381). Results 84.3 % of DS and 80.1 % of RS participants were fully vaccinated. In the DS, after adjusting for demographics and confounders, children with incomplete-vaccinations were at greater risk for ASD when compared to those fully vaccinated (adjusted odds ratio [aOR] = 1.42, 95 % Confidence Interval [CI] 1.17?1.73 with 4?5 vaccinations; aOR = 2.33, CI 1.53?3.56 with vaccination <3). The DS finding was confirmed in the RS (aOR = 1.44, CI 1.32?1.58 with 4?5 vaccinations and aOR = 2.19, CI 1.80?2.67 with < 3 vaccinations). In the DS, those with incomplete-vaccinations were at a greater risk for internalizing and externalizing symptoms. Conclusions We replicate our own and prior findings that vaccination does not increase ASD risk.
