Forty children with ASD and forty age-matched typically developing children between the ages of 7–12 years participated. Groups were comparable with regard to nonverbal IQ, but children with ASD had significantly lower verbal IQ. Children completed three CBT-related tasks requiring emotion recognition, discrimination among thoughts, feelings and behaviors, and cognitive mediation. With the exception of the emotion recognition task, children with ASD performed comparably to typically developing children and with a high rate of accuracy. Given the significant interference and distress that excessive anxiety can cause, evidence-based intervention is necessary in order to reduce long-term negative effects. Cognitive behavioral therapy (CBT) has demonstrated efficacy for treating anxiety disorders across the lifespan, both in individual and group formats. Recently, modified CBT programs for youth with ASD have been developed, showing positive outcomes. To date, these modified CBT programs have primarily been evaluated in controlled research settings. METHODS: The current community effectiveness study investigated the effectiveness of a modified group CBT program (Facing Your Fears) delivered in a tertiary care hospital and across six community-based agencies providing services for youth with ASD. Data were collected over six years (N = 105 youth with ASD; ages 6-15 years). RESULTS: Hospital and community samples did not differ significantly, except in terms of age (hospital M = 10.08 years; community M = 10.87 years). Results indicated significant improvements in anxiety levels from baseline to post-treatment across measures, with medium effect sizes. An attempt to uncover individual characteristics that predict response to treatment was unsuccessful. CONCLUSIONS: Overall, this study demonstrated that community implementation of a modified group CBT program for youth with ASD is feasible and effective for treating elevated anxiety. The present study is a follow-up to a clinical trial that examined the implementation and treatment outcomes in response to a group CBT program for youth with ASD and co-occurring anxiety (i.e., Facing Your Fears; FYF). This study uses a mixed-methods approach to understand: 1) the percentage of clinicians who continue to use FYF at least four years after being trained in the program (i.e., sustainment); 2) adaptations that have been made to FYF in order to sustain its fit within clinical settings; and 3) factors that influence clinicians’ sustained use of FYF. Method Thirty of 34 clinicians who were initially trained to use FYF responded to a mixed-methods survey asking about their sustained use of the program, adaptations made to FYF, attitudes towards FYF, and perceived barriers to continued FYF use. Results Results demonstrated that a majority of clinicians reported that they continue to use the FYF program. Clinicians described that they continue to use FYF due to their perceptions of the program’s effectiveness, ease of use, and compatibility, but also made a variety of adaptations to FYF. Conclusion Results from the present study help to better understand factors impacting the sustained use of CBT for youth with ASD. They also highlight the importance of proactive efforts within clinical training to plan for intervention adaptation and to consider factors likely to impact program sustainment. Trained observers blind to treatment condition observed each child during recess on two separate days at baseline and again at posttreatment, using a structured behavioral observation system that generates frequency scores for observed social communication-related autism symptoms. CBT outperformed TAU at posttreatment on the frequency of self-isolation, the proportion of time spent with peers, the frequency of positive or appropriate interaction with peers, and the frequency of positive or appropriate peer responses to the target child (d effect size range 1.34–1.62). On average, children in CBT were engaged in positive or appropriate social interaction with peers in 68.6 % of observed intervals at posttreatment, compared to 25 % of intervals for children in TAU. Further investigation of this intervention modality with larger samples and follow-up assessments is warranted.
